How do I arrange for home care?

After receiving a referral from your physician, our staff will assign a home care nurse or physical therapist to visit your home to assess your condition, contact your physician to discuss your home care needs and decide on the most appropriate services.

We work very closely with your physician to tailor services to your needs to provide the exact care prescribed. We are in constant touch with your physician throughout your care, keeping he/she up to date on your condition. We also work very closely with you; your family, or caregivers to transition care.

Why should I use home care?

Liberty Home Care is committed to helping you recover as quickly as possible, and we feel that the best way to do that is within the comfort and familiarity of your own home. Studies have shown that patients who are in familiar and relaxing environments, such as their own homes, recover in a timelier manner.

With advancements in technology, we have brought home care to new levels of performance. High tech, highly specialized treatments that use to require patients to be hospitalized can now be delivered just as safely and efficiently right in the patient's home.

With Medicare, managed care companies and other insurance plans requiring shorter hospital stays. Many patients with complex health requirements discover themselves going home sooner. Home care allows you to have the continuity of care you need to recover faster and stay healthy.

Who qualifies for Home Care?
All age groups.
Medicare requires persons to be essentially homebound due to illness or injury.
Persons needing intermittent skilled nursing care, physical therapy or speed therapy.
Persons under the care of a physician.
What insurance do you accept?
Medicare Part A cover Home Health Services.
All Auto Insurance.
All Workers' Compensation Insurance.
Most Private insurance cover Home Health care.
What payment options do I have?
If your services are covered by insurance, we bill them directly. Otherwise, we will bill the paying party on a weekly basis. Invoices are due upon receipt.
Are your employees bonded and insured?
Yes, all of Liberty Homecare's Team members are bonded and insured. They are directly employed by us and we assume all responsibility for their taxes, benefits, workers compensation and insurance.
How soon can service typically start?
Services can begin and end as needed. Hours can increase and decrease as needed. We request a 48 hour notice to end services so we have time to move the Liberty Homecare Team member(s) to other assignments. Liberty Homecare will work hard to customize a care plan that works for the entire family.
What if I no longer need the services?

What is the cost if I am paying for it myself?
Rest assured- Liberty Homecare's services are affordable. An initial phone call can generate an estimate, however specific prices are determined after the Personalized Assessment is completed.
What should you do if you have a concern about, or are not satisfied with the quality of care you received?
Call Liberty Home Care at 1-800-284-7420 and describe your concern. One of our trained staff will listen and help you decide what to do. One of your choices is to make a formal complaint.
If you decide you want to make a formal complaint, we can help you put it in writing. Once Liberty Home Care receives the written complaint, the quality of care review will begin. This is a free service for Medicare members.
Who conducts the review and what does it include?
A nurse review coordinator will contact you when your complaint is received and will serve as your contact person throughout the review process. A Liberty Home Care physician reviewer will review your complaint along with your medical record(s). The physician reviewer is a practicing physician who works with Liberty Home Care to address quality of care concerns.

The physician reviewer looks at all aspects of care surrounding your complaint, and makes an initial finding that is shared with your physician or health care facility that your complaint is related to. Your physician/health care facility has the opportunity to comment. Liberty Home Care works directly with you, your physician and your health care facility to resolve the problem and make sure that you and other Medicare beneficiaries receive quality health care.

The entire process takes anywhere from three to six months.

What areas do you cover?

When you request our home care services, our director of nursing, who oversees all of our home care staff, will work with the patient and physician to make a medical assessment and recommend the level and type of home care needed. Our home health caregivers will visit any home or assisted living facility within Wayne, Oakland, Macomb, Washtenaw, Detroit metro areas & Monroe County. Additionally, all of our caregivers are available 24 hours a day, 7 days a week.

If a patient has a special request, we meet that need to ensure the patient's satisfaction. In short, we cater to our patients' needs. Our large number of staff makes possible almost any type of accommodation.
